EOScu EPA Required Installation Items Request Form

Please Note: 

• Each installed product must have the EPA Required Tag adhered to its surface. 
  • This tag is required to be adhered to every finished product, regardless of the number of installations per room at any facility. 
  • Example: If a patient room includes multiple EOScu horizontal tops, a vanity in the restroom, a sink counter in the room, an over bed table, and bed rails, each item must have a tag adhered.  
• An EPA Required Pamphlet must be left with the on site point of contact at each installation site.
