# EOScu EPA Required Installation Items Request Form

## Please Note: ![EPA Required Items-01](https://info.eoscu.com/hs-fs/hubfs/EPA%20Required%20Items-01.jpg?width=300&name=EPA%20Required%20Items-01.jpg)

- Each installed product must have the EPA Required Tag adhered to its surface.  
    - This tag is required to be adhered to every finished product, regardless of the number of installations per room at any facility. 
    - Example: If a patient room includes multiple EOScu horizontal tops, a vanity in the restroom, a sink counter in the room, an over bed table, and bed rails, each item must have a tag adhered.  
- An EPA Required Pamphlet must be left with the on site point of contact at each installation site.
